Effective May 12, 2017, CHC has switched their Protect Youth Sports screening system to "Auto Screen." With the "Auto Screen" process, ALL applications will be automatically screened by Protect Youth Sports and CHC will be billed for every application, whether or not the applicant was due to be rescreened. We want to avoid having these potential additional costs whenever possible. A list will be provided on this page of officials who completed screenining for the 2018-19 season. If your name is on this list you do not have to re-screen.
Additionally, all applicants who are approved will be transmitted to USA Hockey as "screened" with the date. Actual screening reports will not be transmitted to USA Hockey.
During the screening questions, you will be asked for your USA Hockey Participant Number. This is not your Officiating Number. It is a string of nine numbers and five alphabetic characters and can be found as the last 14 characters in the last link at the bottom of the email you received when you completed your registration.
If your program is not listed in the list of programs you can affiliate with, you must email Kathy Ludwig prior to submitting a screening application so you application can be approved for screening.

FOR THE 2019-20 SEASON:
Officialsscreened for the 2018-19: do not have to be screened and should NOT submit a CHC online application. CHC Screening policy is to screen an individual coach/volunteer/official every 2 years.
Officials screened/rescreened in any season prior to 2018-19 MUST be rescreened, and submit a CHC online application
PLEASE NOTE: Officials, age 17 and under, cannot be screened, nor should they submit applications on the website.

When completing the application online, please double check your entries, especially your gender, date of birth, and social security number. Errors in these entries will cause delays in processing your application, and may result in your having to complete a new application.
